Become a member in 3 easy steps
1. Fill in the online registration form
The registration form has 2 sections; one for your general details and the second part is to list your symptoms of CMTC. This last section is very important for the patient registering but also for the other patients.
All information provided is kept strictly confidential.
2. Transfering the first payment
From the Netherlands you can transfer € 25.00 to bank account 76.17.526 in the name of CMTC-OVM, in Leusden.
For International transfers: Bank Identification Code (BIC): INGBNL2A / IBAN number: NL27INGB0007617526
You can also make use the 'online donation' option. This is a fast and secure
way of paying. As soon as we receive the Registration form and the
Symptoms list, the we will know that the transferred money was not a
donation but for the membership.

Membership
If you choose to become a member of our organisation you will recieve our newsletters which contains important and easy to read information. As a member you can also have contact with other members through our general meetings or during the event organised as part of the yearly patient contact days. Its also possible to contact people from other countries with the help of video conferencing. We have all the knowledge and experience to set this up for you. Membership costs
€ 25.00 per year. In the Netherlands many health insurance companies reimburse the membership fee so that the membership ultimately is free!
If you would prefer to not be a member but to support our organisation then you can also make a donation starting at € 10.00 per year.

Existing members are not required to re-register annually. Each member will receive a confirmation letter after registration which outlines that the membership automatically renews itself. This letter is also states the date at which you need to unregister if you no longer wish to be a member. During the year members receive an invoice for the minimum (or more if you wish) membership fee.
Of course, there is always an option to pay in advance. This saves us time and effort and in some cases even money (eg postage).
