And then the time comes: you are considered an adult in medical care

The transition in care refers to moving from pediatric to adult healthcare. The actual transfer is just a moment, but the transition involves much more. You are prepared in advance to make the move as smoothly as possible. The entire transition phase includes preparation, the transfer itself, and the adjustment period afterward.

Preparation and guidance for the transition can take place at a transition clinic. This is a clinic where you are helped to become increasingly independent in managing your condition. After the transition, you are expected to:

  • Know which medications you take

  • Speak up for yourself during consultations with your healthcare provider

  • Schedule your own appointments with specialists

Fortunately, many hospitals pay attention to this process and have guidance programs in place.
